    99mTc-MIBI Lung Scintigraphy in the Assessment of Pulmonary Involvement in Interstitial Lung Disease and Its Comparison With Pulmonary Function Tests and High-Resolution Computed Tomography: A Preliminary Study

    The differentiation of active inflammatory processes from an inactive form of the disease is of great value in the management of interstitial lung disease (ILD). The aim of this investigation was to assess the efficacy of 99mTc-methoxy-isobutyl-isonitrile (99mTc-MIBI) scans in distinguishing the severity of the disease compared to radiological and clinical parameters.In total, 19 known cases of ILD were included in this study and were followed up for 1 year. Five patients without lung disease were considered as the control group. The patients underwent pulmonary function tests (PFTs) and high-resolution computed tomography scans, followed by 99mTc-MIBI scanning. The 99mTc-MIBI scans were analyzed either qualitatively (subjectively) or semiquantitatively.All 19 ILD patients demonstrated a strong increase in 99mTc-MIBI uptake in the lungs compared to the control group. The 99mTc-MIBI scan scores were higher in the patient group in both the early phase (0.240.19-0.31 vs 0.110.10-0.15, P 0.14). The 99mTc-MIBI scan scores were not significantly correlated with the PFT findings (P > 0.05). In total, 5 patients died and 14 patients were still alive over the 1-year follow-up period. There was also a significant difference between the uptake intensity of 99mTc-MIBI and the outcome in the early phase (dead: 0.320.29-0.43 vs alive: 0.210.18-0.24, P < 0.05) and delayed phase (dead: 0.270.22-0.28 vs alive: 0.100.07-0.19, P < 0.05).The washout rate was ~40 min starting from 20 min up to 60 min and this rate was significantly different in our 2 study groups (ILD: 46.6115.61-50.39 vs NL: 70.9127.09-116.36, P = 0.04).The present study demonstrated that 99mTc-MIBI lung scans might distinguish the severity of pulmonary involvement in early views, which were well correlated with HRCT findings. These results also revealed that 99mTc-MIBI lung scans might be used as a complement to other diagnostic and clinical examinations in terms of functional information in ILD; however, further investigations are strongly required

    Comparison of the Eficacy of Intravenous Magnesium Sulfate and Oral Indomethcin in the Management of Preterm Labor

    Abstract: Background & Aims: Preterm labor is often resulted in preterm births and increased rate of neonatal morbidity and mortality. Treatment consists of bedrest, hydration, pharmacologic interventions, and combinations of these. The purpose of this study was to compare the efficacy of intravenous magnesium sulfate (MgSO4) and indomethacin in the treatment of preterm labor pains. Neonatal and maternal side effects of each method were also studied. Methods: A total of 120 women between 26 and 31 weeks’ gestation admitted due to preterm labor were randomized to receive either MgSO4 (n = 60) or oral indomethacin (n = 60). All women received betamethasone and prophylactic antibiotics. Data were analyzed using student t-test and X2. Results: The two groups had no significant difference in mean gestational age and cervical dilation and effacement at enrollment. The gestational age at delivery was similar in both groups (p=0.279). Delivery was delayed for more than 48 hours in 81% of subjects in MgSO4 group and in 87% of subjects in the indomethacin group (P=0.298). Neonatal birth weight, type of delivery, recurrent preterm labor pain and mean time to suppress the contractions were similar in both groups. No serious side effect was seen in the two groups. Conlusion: Acute tocolysis by either IV MgSO4 or oral indomethacin can delay preterm birth for 48 hours, provide the critical period of steroid effect, arrest an episode of preterm labor, and consequently delays delivery and improves neonatal outcomes. It is therefore necessary that all Obstetricians remain up-to-date regarding the efficacy, indications, contraindications, and side effects of all tocolytics.     Comparing Carotid Intima-Media Thickness in Women with and Without Gestational Diabetes Mellitus

    Abstract: Background & Aims: Women with history of gestational diabetes mellitus (GDM) are at risk for developing cardiovascular disease and metabolic syndrome. Carotid intima-media thickness (CIMT) is noninvasive marker of sub clinical atherosclerosis assessment. The aim of the present study was to assess relationship between GDM and increased CIMT. Methods: Twenty women with previous history of GDM and twenty women without it (as a control group) were included in the study. Two groups were matched based on their age, body mass index (BMI) and parity. CIMT was measured one year after delivery in two groups by B-mode sonography and compared with ultrasonic results of CIMT in mid- and late-gestational periods. Results: CIMT in both groups increased in gestational period and decreased after delivery and this diminution was significant at late-gestational period in both groups and also significant at mid-gestational period in women with history of GDM.     Empirical constraints on the origin of fast radio bursts: volumetric rates and host galaxy demographics as a test of millisecond magnetar connection

    The localization of the repeating FRB 121102 to a low-metallicity dwarf galaxy at z=0.193z=0.193, and its association with a quiescent radio source, suggests the possibility that FRBs originate from magnetars, formed by the unusual supernovae in such galaxies. We investigate this via a comparison of magnetar birth rates, the FRB volumetric rate, and host galaxy demographics. We calculate average volumetric rates of possible millisecond magnetar production channels such as superluminous supernovae (SLSNe), long and short gamma-ray bursts (GRBs), and general magnetar production via core-collapse supernovae. For each channel we also explore the expected host galaxy demographics using their known properties. We determine for the first time the number density of FRB emitters (the product of their volumetric birthrate and lifetime), RFRBτ≈104R_{\rm FRB}\tau\approx 10^4Gpc−3^{-3}, assuming that FRBs are predominantly emitted from repetitive sources similar to FRB 121102 and adopting a beaming factor of 0.1. By comparing rates we find that production via rare channels (SLSNe, GRBs) implies a typical FRB lifetime of ≈\approx30-300 yr, in good agreement with other lines of argument. The total energy emitted over this time is consistent with the available energy stored in the magnetic field. On the other hand, any relation to magnetars produced via normal core-collapse supernovae leads to a very short lifetime of ≈\approx0.5yr, in conflict with both theory and observation. We demonstrate that due to the diverse host galaxy distributions of the different progenitor channels, many possible sources of FRB birth can be ruled out with ≲10\lesssim 10 host galaxy identifications. Conversely, targeted searches of galaxies that have previously hosted decades-old SLSNe and GRBs may be a fruitful strategy for discovering new FRBs and related quiescent radio sources, and determining the nature of their progenitors

    Anti-melanocortin-4 receptor autoantibodies in obesity

    Background: The melanocortin-4 receptor (MC4R) is part of an important pathway regulating energy balance. Here we report the existence of autoantibodies (autoAbs) against the MC4R in sera of obese patients. Methods: The autoAbs were detected after screening of 216 patients' sera by using direct and inhibition ELISA with an N-terminal sequence of the MC4R. Binding to the native MC4R was evaluated by flow cytometry and pharmacological effects by measuring adenylyl cyclase activity. Results: Positive results in all tests were obtained in patients with overweight or obesity (prevalence: 3.6%) but not in normal weight patients. The selective binding properties of anti-MC4R autoAbs were confirmed by surface plasmon resonance and by immunoprecipitation with the native MC4R. Finally it was demonstrated that these autoAbs increased food intake in rats after passive transfer via intracerebroventricular injection. Conclusion: These observations suggest that inhibitory anti-MC4R autoAbs might contribute to the development of obesity in a small subpopulation of patients

    In vitro propagation of four Iranian grape varieties: Influence of genotype and pretreatment with arbuscular mycorrhiza

    There is a great demand for table grape saplings, mainly for commercial varieties indicating that micropropagation could be an effective method for their mass propagation. Internal contamination in woody plant species is an important problematic issue and arbuscular mycorrhizal fungi (AMF) have been known as potential plant biological protectors. In the present study, the glasshouse grown mother plants of four grape varieties ('Asgari', 'Khalili', 'Keshmeshi', and 'Shahroudi') were inoculated with AMF as pre-treatment. The fungi strains were Glomus mosseae, G. fasciculatum, G. intraradices and a mixture of all three species. The comparative in vitro performance of these genotypes was evaluated following optimization of in vitro growth conditions for each genotype. Furthermore, the positive effect of AMF inoculation of stock plant on micropropagation process was studied. Changes in biochemical features (total chlorophylls, total phenols and total sugars), growth parameters (root length and total leaf area) and in vitro behavior of AMF pretreated as well as control explants were recorded. The mycorrhizal association with grapevine roots was confirmed following root staining and evaluation of colonization rate. The results revealed a distinct difference and clear genotypic effect on various in vitro parameters of studied grape genotypes. The utilized inocula were found to have the capability of mycorrhizal association with grapevine roots, leading to enhancing phenolics as a defense mechanism, increasing sugars and chlorophyll and finally growth of whole plant corresponding to the grape variety and AMF strain. These results confirmed that health and physiological conditions of the stock plants are important parameters for in vitro grape culture establishment and suggest the integration of mycorrhizal technology with tissue culture to accomplish better results

    Increased sample asymmetry and memory of cardiac time-series following endotoxin administration in cirrhotic rats

    Sepsis, and other causes of acute systemic inflammation, can reduce heart rate variability (HRV) and increase cardiac cycle regularity in mammals. Thus, HRV monitoring has been used for early detection of sepsis in adults and neonates. Liver cirrhosis is associated with reduced basal HRV and the development of tolerance to the cardiac chronotropic effects of bacterial endotoxin. This may pose limitations on the use of heart rate monitoring in early detection of sepsis in this patient population. In a study to develop a physiomarker for the detection of sepsis in cirrhosis, we observed that endotoxin administration in adult cirrhotic rats leads to the development of transient heart rate decelerations, a phenomenon which has been reported in neonates with sepsis, and quantified using sample asymmetry analysis. In the present study, cirrhosis was induced by surgical ligation of the bile duct in rats. Cirrhotic rats were given intraperitoneal injections of either saline or endotoxin (1 mg kg(-1)). Changes in sample asymmetry and memory length of cardiac time-series were studied in conscious rats using implanted telemetric probes. Cirrhotic (but not control) rats exhibited increased sample asymmetry following endotoxin injection, which was consistent with the development of transient heart rate deceleration. Endotoxin administration in cirrhotic rats was associated with prolongation of memory length for observing decelerating perturbations in the cardiac rhythm. These findings may have application in the development of an HRV monitoring system for early detection of sepsis in cirrhosis
